Poliovirus and Poliomyelitis Practice Test

Protection against poliomyelitis relies on two coordinated immune responses that address both the gut where the virus first replicates and the bloodstream/nervous system where disease can develop. First, neutralizing antibodies must target all three poliovirus serotypes. Each serotype is distinct, so antibodies against one do not neutralize the others. If antibodies are present for only one serotype, infection with the other serotypes can still occur and potentially lead to paralysis.

Second, mucosal IgA in the intestinal lining provides a barrier at the entry site, reducing gut replication and, importantly, limiting virus shedding and person-to-person transmission. This intestinal or mucosal immunity is crucial because poliovirus begins in the gut after ingestion and the mucosal response helps prevent both disease and spread.

T-cell memory alone cannot prevent initial infection at the gut, and antibodies limited to a single serotype leave a person vulnerable to other serotypes. And without specific immunity, there would be no targeted defense against the virus at all.
